CareCo Medical is recruiting per diem Connecticut Certified Home Health Aides (CNA) to join our Home Health Care Agency. We are looking for motivated self-starters and people who are ready to begin working with our expanding team to provide top-quality home care services to our clients in the New London County area.

Home Health Aide Duties & Responsibilities:

• Follows care plan
• Provides personal hygiene and grooming care 
• Report changes in clients' condition to supervisor
• Provide reminders for prescribed medications and supplements
• Communicate with the care team and families
• Complete visit care plan note for each visit timely, within agency guidelines
• Assist patients with mobility and physical therapies/exercises
• Perform light domestic and household tasks

Home Health Aide Requirements:

• Licensure/Certification: Home Health Aide or CNA Certificate approved by the Connecticut Department of Public Health

• Caring and compassionate personality
• Current CPR certificate
• First Aid training
• Valid drivers license & active auto insurance required
• Reliable personal vehicle required
• High school degree
• Experience as Home Health Aide/CNA in a home care agency, preferred

Pay: $16-$18 per visit. 

Fulfill the role of liaison between patient/family, other health care providers, and our home care cross-functional care team to ensure necessary care is provided promptly and effectively.
