Care360 Hospice is Hiring a Hospice Nurse (Afterhours) Near Dayton, OH

Hospice Nurse 

LPN Salary: $65k-70,096/yr BOE

RN Salary: $80-89k/yr BOE

Schedule: Full-time| Mon-Thurs or Fri-Mon | after-hours/OC

Location: Springfield Ohio and surrounding areas

Requirements of the Hospice RN/LPN role:

  • Licensed LPN/RN in state of practice
  • CPR/BLS Certified
  • A minimum of 1 year of Clinical experience
  • Hospice experience preferred
  • Valid Drivers License and Auto insurance Coverage
  • Reliable transportation

About the Hospice RN/LPN role:

  • Provide skilled nursing care
  • Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
  • Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders. 
  • Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed. 
  • Assist reinforcement of patient and family education regarding end-of-life care.
  • Participate in IDG and case conferences
  • Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning. 
  • Keep the RNCM informed of changes noted during the process.
  • Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
  • Respond to on-call or medical emergencies (nights and weekends)
