Care with LOVE, a home health service that seeks to improve the quality of life through dependable and caring service, is recruiting skilled care LPNs who are interested in working in Gainesville, VA and surrounding areas.

The skilled clinician plans, organizes, and directs home care services and is experienced in their field, with an emphasis on community health education/experience. The skilled clinician builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

The Clinician we are looking for should be able to:

1. Provides direct patient care.

2. Implements a plan of care created by the registered nurse.

3. Provides accurate and timely documentation consistent with the plan of care.

4. Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.

5. Participates in coordination of home health services, appropriately reporting the identified

needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or

Clinical Supervisor.

6. Uses equipment and supplies effectively and efficiently.

7. Participates in personal and professional growth and development.
