Position Summary

Essential Duties and Responsibilities

• Assist in organizing social services to meet needs of patients in the nursing home
• Assist in interviews of patients referred to the Consultant by the Administrator, Director of Nurses, or Attending Physician.
• Complete necessary forms and makes arrangements for assistance whenever possible and involves the family or sponsor when planning for discharge or recovery programs.
• Makes arrangements for transportation to other institutions or facilities when necessary.
• Provides information to patient and family in regard to Medicare, Medicaid, and other welfare agencies concerning Social Services.
• Obtains information concerning the patient’s personal and family problems, past history of illness’ and care by developing a Social Service Data Form.
• Maintains regular progress notes indicating response to the treatment plan and adjustment to nursing home life. Progress notes shall be recorded on the Common Progress Notes attached to the Inter-Disciplinary Assessment Form, on a current basis.
• Compiles information about community health and social services available and keeps up-to-date resource file on these community services.
• Makes referrals to referral agencies when requested and appropriate, keeping records of such referrals, and provides the referral agency a social service summary.
• Makes written reports to the Administrator concerning social services on a monthly basis.
• Assist discharged patients in securing further assistance when necessary.
• Advises the Administrator of all legislative changes, programs, and regulations.
• Provides in-service training to patient care personnel on a semi-annual basis.
• Others as directed by the Administrator.
