Clinical Duties
Reviews assigned provider tasks including:
Hospitalization notifications, prescription refills, consultant notes, laboratory and diagnostic imaging and report results
Obtains prior authorization for services and medications and/or communicates need for authorization
Communicates with patients and creates chart updates for provider signature
Provides direct patient care and minor office procedure surgical assist as needed
Provides nursing triage as needed including:
Emergency, isolation, and crisis intervention, administer medications as needed
Supports the Office Manager in overseeing Patient Care Assistants (PCAs) during absences and/or when needed
Supports medical management with daily coordination, direction, and evaluation of delivery of patient care for the health center’s medical care department.
Provides and promotes quality patient care based on the individual needs of the multicultural patient populations served
Serves as liaison between medical providers, medical support staff and other departments.
Assists in the coordination of care with the interdisciplinary team members for achievement of patient goals.
Administrative Duties
Handles all communications including:
Daily email, telephone calls/information requests with courtesy, accuracy, and respect for confidentiality; receives information and distributes messages as needed.
Responsible for accurate and timely documentation in EHR.
Participates in internal committees, meetings, audits, site visits as needed.
Gathers patient information accurately and completely by utilizing appropriate templates in agency’s database:
Current Electronic Health Records (EHR), Practice Analytics, Provide.
Quality Assurance/Compliance
Adheres to Infection Control Policies and Procedures, including Universal Precautions in order to achieve compliance and maintain a safe environment of care.
Maintains agency guidelines relating to safety, outreach and confidentiality.
Ensures proper hand washing according to Centers for Disease Control and Prevention guidelines.
Understands and appropriately acts upon assigned role in Emergency Code System.
Understands and performs assigned role in Agency Continuity of Operations Plan (COOP).
Ensures that medical operations fully comply with Agency and HIPAA requirements.
Reviews patient’s records on a monthly basis for quality assurance compliance.
Promotes agency’s ancillary services (i.e. Nutrition, Dental, Behavioral Health, Specialty Care) when clinically indicated and appropriate.
Ensures that online trainings are current as required by the health center medical curriculum.
Assists supervisor in the training of new staff (peers).
Conducts quarterly assessment skills for LPNs and Patient Care Assistants

Physical Requirements
This work requires the following physical activities: constant talking in person, talking on the phone, hearing/visual acuity and exposure to blood borne pathogens and hazardous materials. Frequent, walking, sitting and bending. Occasional standing, driving and climbing may be required. Work is performed in clinical/office settings.
Other
Participates in health center developmental activities as requested.
Other duties as assigned.
Education:
Bachelor’s Degree in nursing or healthcare related field required.
Training and Experience:
At least one year of related work experience required. She/he must have knowledge in promoting wellness and the management of chronic diseases/conditions within the primary care setting. Trainings in HIV/AIDS are recommended.

Bilingual (English Spanish) is preferred. Computer knowledge should include Microsoft Office, Electronic Health Records (EHR) and related software programs. S/he must possess technical skills, communication, critical thinking, interpersonal skills, flexibility, organizational skills, accountability, and leadership. Excellent organizational, presentation, communication, problem-solving and teamwork skills are required. Ability to work independently and handle stress appropriately. Ability to work cooperatively with medical, pharmacy and other supporting staff. Ability to work with multicultural, underserved, and diverse population is required.
