Essential Job Responsibilities
Outreach and Linkage
Identifies patients, whether they come in on their own to the clinic for testing, or directly to the medical desk of one of our offices, and assists them in the process of getting registered to engage with the medical provider and pharmacy for PrEP.
They may identify clients who have been lost to care, recently diagnosed with an STI, recently seroconverted partner, who may have had a recent exposure to HIV (PEP) or who needs a new provider, and assist them to maintain and adhere to their PrEP protocol or access to PEP.
Facilitates a welcome into the agency and either provides or facilitates orientation/education regarding PrEP and the agency and its various services.
Maintains an open door policy to help clients keep their medical, mental health or other appointments and remain adherent to their protocol.
Coordinates with the pharmacy to have the client registered with the appropriate assistance program whether they are insured or uninsured.
Advocates for patient to oversee the process of intake through to completion with the delivery of medication in hand when dealing with PEP clients.
Follows up with PEP clients to transition to PrEP if that is their choice.
Maintains accurate data on all client interactions and coordinates logs with the Clinical Manager.
Communicates with clients one to three days before a foreseen hurricane/natural disaster according to emergency plan procedure.
Health Education and Navigation
Provides information and support to patients through shared experiences using encouragement, modeling behavior, various interviewing techniques to reinforce healthful behaviors and create better medical outcomes.
Provides accurate referrals to patients who have mental health, substance abuse and homelessness issues.
Serves as mentor and advocate or liaison regarding interaction with medical providers, helping patients prepare for visits and then debriefing what happened, depending on patient need.
Prepares the client thoroughly by provides educational support to clients to enhance their knowledge and understanding of medication protocols, side effects and adherence to help them achieve a better medical outcome.
Participates in staff training sessions and other meetings as required by the agency and/or the funding sources.
Collaboration
Provides culturally competent peer navigation services in a variety of community settings.
Acts as support staff in the implementation and facilitation of group sessions provided by the Care Resource providers.
Documentation, Compliance and Quality Assurance
Maintains Timely, Accurate, Legible and Clear chart documentation according to agency and funder requirements.
Maintains record keeping requirements and assists with chart reviews for Q/A purposes as requested.
Provides supervisor with information needed for end of month reporting and/or participates in program evaluation as requested.
Assists in collecting client satisfaction survey data from assigned patients as requested

Contact Responsibility:
The responsibility for external contacts is frequent and important.
Physical Requirements:
This work requires the following physical activities: constant talking in person, talking on the phone, hearing/ visual acuity. Frequent driving, walking, sitting and bending. Occasional standing and climbing are required. Work is performed in an office or community settings.
Other
Participates in health center developmental activities as requested.
Other duties as assigned.
