Care Partners Hospice is Hiring a Hospice LPN Near Laguna Niguel, CA

Responsibilities:
- Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Conduct assessments and document patient progress.
- Perform case management duties, including coordinating referrals and ensuring continuity of care.
- Assist with airway management and respiratory treatments as needed.
- Work with individuals with developmental disabilities to promote their health and well-being.
- Provide pain management interventions and monitor patient comfort levels.
- Administer inhalation therapies as prescribed by physicians.

Requirements:
- Valid LPN license in CA
- Strong knowledge of occupational health practices and regulations.
- Excellent communication and interpersonal skills.
- Strong organizational skills and attention to detail.

Pay: From $55.00 per hour
