What you will do:

• Provide comprehensive care and skilled nursing to patients in their home.
• Conduct assessments and under the guidance of the supervising physician, develop plans of cares for patients, utilizing nursing skills and techniques to provide ongoing and quality care to the patient.
• Deliver case management and follow up skilled nursing visits with each patient assigned.
• Engage family members and caregivers, maintaining communication about the care plan, providing training and education to ensure consistency with care.
• Assess medications to identify possible ineffective therapy or adverse reactions, side effects, drug allergies, or contraindicated medications.
• Provide ongoing communication with other members of the patents’ multidisciplinary team, including Home Health Aides, Physical Therapists, Occupational Therapists and other external healthcare providers.
• Maintain thorough and accurate documentation on electronic medical records, ensuring seamless course of care and compliance with all regulatory requirements.
• Other duties as assigned

What you will need:

• Valid RN license in state of Louisiana
• Valid driver’s license, auto insurance and reliable vehicle
• Minimum one year of clinical experience as a Registered Nurse (RN)
• Current CPR certification
• Strong attention to detail, time management and decision-making skills
• Demonstrated timely, consistent job attendance history
• Must be comfortable working in a fast-paced environment
• Commitment to patient quality as a top priority
• Flexible and proactive to meet varied patient needs
• Self-motivated and able to work autonomously to ensure completion of tasks
• Able to collaborate with other healthcare professionals

Pay: $30.00 - $40.00 per hour
