The following does not imply that these are the only duties required. Other duties may be assigned as deemed necessary and appropriate.

• Meet with individuals/families residing at Care Corps LifeHouse to cooperatively develop a case plan using specific, personalized goals that will enable each individual/family to achieve self-sufficiency.
• Meet with each individual, family, and youth residing in Care Corps LifeHouse’s Supportive Housing Programs to work cooperatively to develop a case plan using specific, personalized goals that will enable each program participant/family to achieve self-sufficiency.
• Assist in applying for non-cash benefits if applicable.
• Ensure children are registered, receiving transportation to and attending school. Communicate with schools and school counselors and coordinate services as necessary.
• Develop case plans, communicate goals with staff team and Director of Housing & Case Management, monitor progress, and revise/update as needed.
• Monitor, coach, counsel and supervise individuals/families as they work to meet the goals of their case plan.
• Collect supporting documentation from program participants.
• Network with collaborating agencies to coordinate services for program participants and foster positive relationships in the service community. Accountable for communication effectiveness that creates a cooperative environment.
• Documentation, data, and case notes entered, maintaining compliance with Care Corps, Inc.’s policies and procedures. Maintain up-to-date and accurate program participant files.
• Monitor outcomes of caseload.

Pay: $18.00 - $19.00 per hour
