Aspen Trace is now seeking an Transitional Care Nurse (TCN-RN only) The purpose of the Transitional Care Nurse (RN) is to focus on the management of chronic disease and conditions within the facility to reduce hospital readmission rates in accordance with accepted standards of practice, state and federal regulations, and operational policies and procedures. EDUCATION, QUALIFICATIONS, CREDENTIALS: Must be a Registered Nurse within the State of Indiana. Experience in a long term care and/or case management experience, preferred. Certification as a Case Manager, preferred.
