About Cardea Health

Cardea Health is an Oakland based nonprofit healthcare organization founded to connect marginalized populations to the clinical and supportive services they need to improve their health, remain stably housed in their community, and age in place. Cardea Health’s programs fill a critical – and growing – gap in the system of care. Among people experiencing homelessness, a growing number are living with complex medical needs. They face poor quality of life, increased morbidity and mortality, and medical and social destabilization. But they have few housing options: they need a higher level of care than what is offered through permanent supportive housing, but they do not wish to live in an institutional setting and have limited access to hospice or palliative care.

Cardea Health specifically targets this vulnerable population, providing a range of clinical services within permanent supportive housing sites to both address their medical needs and allow them to live independently in their communities, addressing a critical need within the system of care.


About this role

The Kelly Cullen Community is located in the Tenderloin neighborhood of San Francisco and is home to 174 individuals with extremely low incomes, including those that experienced homelessness and are currently living with health challenges. On-site social services are run by the Tenderloin Neighborhood Development Corporation, clinical services are provided by Cardea Health.

The CNA/HHA provides essential care and support to patients under the supervision of registered nurses (RNs) or licensed practical nurses (LVNs). Their responsibilities include assisting patients with activities of daily living such as bathing, dressing, and grooming, monitoring vital signs, feeding patients, and helping with mobility. CNAs/HHA’s also provide emotional support to patients and report any changes in patient condition to the nursing staff. They play a crucial role in ensuring patients' comfort, safety, and well-being in healthcare facilities.

Basic Qualifications:

  • High School Diploma or equivalent

  • Must be authorized to live and work in the U.S.

  • Must be 18 years of age or older.

  • Must have personal, reliable transportation to and from work sites and shifts.

  • Two years of experience in patient care.

  • Self-directed and able to work with minimal supervision.

  • Detailed oriented and highly organized.

  • Compliance with all company policies.

  • Must provide proof of COVID vaccination or willing to be vaccinated.

  • Prior experience with homeless or incarcerated populations is highly desirable.

  • Proficiency in Microsoft Outlook, Word and Excel is required.

  • Applicants may be required to take a background check after a conditional offer is made. Pursuant to State and local Fair Chance Ordinances, we will consider employing qualified applicants with arrest and conviction records.

  • Current, active Home Health Aide and CNA certification. Candidates with only a CNA license may be considered, contingent on completing an HHA certification within 120 days of hire.

  • Other duties as assigned.


Essential Duties and Responsibilities:
The CNA/HHA will assist clinical staff and shelter residents in the coordination of medical and behavioral health care. Duties include:

  • Conduct patient/personal care as directed by nursing staff and in accordance with resident plan of care.

  • Support resident relationship with primary care and other professional disciplines.

  • Assist nursing team in communication with residents including arranging and facilitating appointments with nursing staff.

  • Ensure adherence to infection control standards on site among staff and residents.

  • In collaboration with housing agency, ensure daily wellness of residents. Participate in immunization programs at the KCC.

  • Assist in the execution of population health and safety interventions on site.

  • Answer calls to nursing station and respond independently to when appropriate.

  • Assist nursing staff with light clerical duties including filing and making copies.

  • Ensure that clinical documentation is current, timely, and accurate.

  • Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

  • Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation.

  • Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect.

  • Other duties and responsibilities as required.

Physical Requirements:

  • Stand constantly.

  • Walk frequently.

  • Reach and twist/turn with hands and arms.

  • Climb or balance and stoop, kneel, crouch or crawl.

  • Frequently lift and/or move up to 20 pounds.

  • Occasionally lift and/or move up to 50 pounds.

  • Vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to focus.

Work Environment:

  • Employees have direct contact with clients/patients while administering care.

  • This position will require traveling to other office locations or off-site meetings as required by the job.

  • Employees may be occasionally exposed to toxic or caustic chemicals.
