Carbon Health is looking for a Behavioral Health Care Manager to serve as a member of a multidisciplinary health care team for the purpose of optimizing mental health. in this role on the Collaborative Care Team, you will help build and implement the new mental health and behavioral health programs at Carbon. Utilizing the Collaborative Care Model we provide behavioral health care that is brief, goal oriented, and measurement-based. With our unique approach to treatment, we aim to help our patients establish trust that meaningful progress/change can be made towards their mental health concerns within a clearly identified timeframe. We collaborate closely with the Primary Care team to assist patients with physical and mental health goals.
Licensure in the state of California OR maintains current and unrestricted license to practice independently (LCSW/LICSW, LMHC, LMFT, LPC or equivalent)
