Capital Hospice & Palliative Care is Hiring a Hospice Community Liaison Near Park Ridge, IL

Capital Hospice & Palliative Care is seeking an experienced Hospice Clinical Liaison. The ideal candidate will be a strategic thinker, driven by meeting and exceeding sales goals and quotas.

At Capital we positively impact end of life by providing client centered, quality, and comprehensive care to those seeking hospice care services. Our team approaches care with dignity, respect, and compassion.

Capital Hospice & Palliative Care supports patients and families in Chicago and surrounding suburbs covering five counties; Cook, Will, DuPage, Kane, and Lake.

Job Responsibilities:

Represents and promotes Capital Hospice services to healthcare professionals in nursing homes, long term care facilities, assisted living, hospitals, home health agencies and physicians in private practice.

Develop and implement programs to raise awareness and educate referral sources and the community on hospice services.

Meet the monthly referral quota by identifying opportunities for formal or contractual relationships with physicians, facilities and organizations.

Works with referral sources to ensure that a new patient successfully begins start of care.

Implements sales plan and coordinates with clinical leadership to ensure an excellent end of life experience.

Job Qualifications:

Bachelor’s Degree in related field

One year experience in hospice, home health, or healthcare sales/marketing with demonstrated growth results preferred

Excellent verbal/written communication and interpersonal skills

Strong time management skills

Proven ability to manage a sales territory and contact database

Valid and current auto insurance and driver’s license

Job Type: Full-time
