Job Description Summary

The Clinical Case Manager is responsible for ensuring that patient care and services is coordinated, delivered, and managed appropriately.

Essential Job Functions/Responsibilities

  • Reviews case referrals. Reviews available patient information related to case, including disciplines required to assist in determining home care needs.
  • Assists with assignment of appropriate clinicians to case.
  • Coordinates and oversees patient services provided by clinical agency personnel.
  • Provides help in assessment, planning, implementation and evaluation of patient and care to all clinical personnel as indicated.
  • Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Communicates regularly with contracted therapy staff to assure appropriate home care plan of care and appropriate delivery patient services.
  • Conducts record reviews and communicates findings and recommendations to Clinical Director and appropriate agency personnel.
  • Assists clinical supervisors in managing clinical teams.
  • Attends case conference meetings with agency personnel to facilitate coordination of care.
  • Assists in the orientation of new agency personnel.
  • Assists Clinical Director in the planning, implementation and evaluation of in-services and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Complies with accepted professional standards and principles.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of agency personnel and, maintains compliance to home care standards.
  • Stays informed about changes in the field of nursing and home health care; shares information with appropriate agency personnel.
  • Performs other duties and activities as delegated.

Position Qualifications

  • Registered Nurse with current licensure to practice professional nursing in the State.
  • Graduate of accredited school of nursing.
  • Three to five year’s clinical nursing experience.
  • Prefer two years supervisory experience in health care setting, preferably home care.
  • Complies with accepted professional standards and practice.
  • Must be a licensed driver with an automobile that is insured in accordance with state and Organization requirements and is in good working order.
  • Demonstrates excellent observation, verbal and written communication skills.
  • Self-directed with the ability to work with little supervision; has good organizational skills.
  • Possesses and maintains CPR certification.
