Cape Regional Health System is Hiring a SOCIAL WORKER/DISCHARGE PLANNER Near Cape May, NJ
As the Social Worker/Discharge Planner, you will:
- Identify and coordinate patient care needs in anticipation of the next site of care after hospitalization.
- Provide guidance for clinical coordination, and collaboration among multidisciplinary caregivers.
- Complete comprehensive psychosocial assessment and review clinical needs of patients as they transition to the next level of care.
- Collaborate with physicians, nurses, and the care team to foster a coordinated approach to discharge planning.
- Utilize clinical knowledge to participate in treatment plan discussion with clinical teams.
- Identify and connect patients to services and resources available to them.
- Communicate with Utilization Review staff on any denials, issues, or barriers to discharge.
Qualifications Include:
- BSW or RN
- Minimum of 1- 3 years of experience in diverse clinical settings
- Licensed in the State of NJ
- Active American Heart Association Basic Life Support (BLS) certification
- Prior case management or discharge planning experience preferred
- Prior experience with Cerner EMR and All Scripts is preferred.
- Working knowledge of federal, state, and local laws that govern health care and case management.
- Knowledge of community resources.
