Position Summary: 

The Care Navigator will enhance the patient experience and promote greater continuity of care by:

• Directly assisting patients with scheduling appropriate follow-up visits before their discharge from the emergency department
• Educating patients on the importance of seeking follow-up care and being engaged in their care journey
• Recording basic data regarding each patient interaction (e.g., whether the patient was able to schedule an appropriate follow-up visit)
• Helping ensure patient follow-up by providing patient reminders (e.g., loyal, phone call)

The Care Navigator will be stationed in the Emergency Department and work in collaboration with the Emergency Department team, Provider staff, and others as needed. Applicants must have a customer service-oriented and patient-centric attitude, effective planning and organizational skills, the ability to be accountable for quality customer service and respond appropriately to a variety of personalities and situations, and ability to maintain patient confidentiality.

Minimum Qualifications:

• High school or GED equivalent preferred; college degree desired
• 2-3 years customer facing and/or clerical experience, preferably in a clinic, physician's office, or other medical setting
