Job Summary

The Clinical Navigator position has responsibility for breast health. The Clinical Navigator will work closely with the mammography department to provide care coordination, case management, education, consultation, and referrals to patients with diagnostic mammo patients in need of further diagnostic procedures, biopsy, genetic testing, and education. The Clinical Navigator will also be a liaison for the High Risk for Breast Cancer Assessment Program to ensure completion of family history questionnaires, analysis of data, patient education, and referrals to the High Risk Breast Clinic and/or Genetic Counselor. 

Essential Functions

  • Meets with BIRADS IV patients to assess level of understanding, discuss diagnostic recommendation of radiologist and coordinate with primary care provider to schedule a surgical consult. Consult with breast cancer patient when needed to give resource information to assist the patient to decide on a treatment plan. Collaborates with a multidisciplinary team to manage the patient’s initial care Provide patient and physician support for procedures. Assess and monitor patient’s condition as required. Initiate appropriate intervention as needed. Promotes and supports opportunities for participation in clinical trials. Completes patient history and performs initial and ongoing patient assessment
  • Reviews patient chart and discusses with providers to identify needs for additional diagnostic follow-up and provider consultation appointments. Coordinates scheduling of tests: labs, x-rays; coordinates consult and collaborates with patient to schedule and explain all needed appointments. 
  • On the day of each encounter, fully documents assessments, referrals, patient progress, and known barriers to care in the appropriate section of the Electronic Medical Record. 
  • Educates patients and families on their diagnosis, prognosis, treatment status, and symptom management. Actively participates in outreach/community education and/or early detection/screening events.
  • Reviews patient charts to identify needs, provides guidance in scheduling necessary diagnostic tests, and obtains the results of these tests. Appropriately triages telephone calls from the patient with problems or complaints. Maintains written and oral communication with statewide home health agencies, referring appropriate cancer patient to these agencies for emotional support, reinforcement of wound care instructions, and/or nutritional and physical assessment. 
  • Stays current with NCCN guidelines for genetic testing for assigned diagnostic group, alerting providers to potential needs and opportunities for scheduling testing or referrals. Educates patient and family regarding molecular/genetic testing and refers to Certified Genetic Counselor and/or High Risk Program as appropriate.
  • Takes initiative in ensuring that needed patient services are available or by referral. Works with health system and community leaders to develop programs and resources as needed.
  • Takes advantage of of continuing education opportunities. Maintains all required certifications. Participates in grand rounds, cancer conferences, and other clinical learning opportunities.

Education, Training, and Experience

Bachelor’s Degree preferred. Three years of experience as a Registered Nurse or Registered Mammography Technologist required. One year of Case Management or Navigation.

Other Job Requirements, Licenses, Certifications

Registered Nurse (RN) in the State of Georgia required OR Registration by the American Registry of Radiology Technologists with certification in both Radiology and Mammography (meeting all MQSA/FDA requirements to perform Breast Imaging). Healthcare Provider BLS/CPR required.
