Job Description:
The Referral Clerk/Care Coordinator Manager would be responsible for the leadership and direct supervision of the Care Coordinator and Referral Clerk team. In addition to the supervisor responsibilities, the Manager would also be responsible for coordination duties for assigned Medical Providers.
Care Coordination is the coordination of patient care and services, ensuring that patients receive the appropriate care and resources to meet their healthcare needs. The Care Coordinators work closely with healthcare providers, patients, to schedule follow up visits from hospital discharges and emergency room visits, follow up with patients to ensure quality care is maintained for client outcomes.
