Overview:
This individual is responsible for the overall healthcare coordination and oversight of all aspects of resident care and actions of the Clinical Care Management Team in accordance with current Federal, State, and local regulations as well as community policies and procedures.
Job Description:
• Participating in a community survey conducted by the department of health as necessary or as directed.
• Maintaining a cooperative and productive relationship with the VP of Operations, Executive Director of Admissions & Compliance, together with the Clinical Care Team, to follow-up and ensure resolution to all identified issues.
• Participation in the Quality System Tracer Review meeting and developing/implementing appropriate plans of action to correct identified deficiencies.
• Assisting in planning for resident admissions and discharges as needed.
• Together with the Administrator and assistance from HR, providing mentoring, training, and oversight to the clinical care staff as their job responsibilities increase and ensuring that proper documentation is completed/uploaded into residents HR file.
• Training and delegating tasks to CBRF Medication certified Clinical Care Assistants as deemed appropriate.
• Assisting with monitoring of the staffing needs of the clinical care staff and ensuring the assignment of appropriate and adequate numbers of clinical care staff daily for each shift to ensure that the needs of the residents are met.
• Performing daily rounds when at the community to monitor and evaluate resident care, proper medication storage area, equipment, and shift report is being completed.
• Assisting fellow team members in maintaining a clean, homelike environment that is free from hazards by tidying up spaces, cleaning up after residents, and ensuring that they always look presentable.
• Preparing, planning, and adhering to the clinical care services budget while ensuring that there are adequate supplies for resident care by monitoring use, performing supply audits, and ordering, when necessary, but remaining within department budget.
• Ensuring that medical and clinical care is administered in accordance with the resident's wishes and per the ISP.
• Planning, attending, documenting, participating in, and reviewing care conferences and service plan reviews.
• Completing all notifications/communications to family, physicians, MCOs, and outside sources as resident care needs change, incidents occur, etc.
• Assisting all outside providers with answering questions while at the community to ensure all major concerns are addressed.
• Maintaining the confidentiality of all resident care information.
• Ensuring residents are treated with dignity and respect and resident rights are maintained.
• Acting as a liaison between the residents, their families, and support departments to adequately plan for the resident's needs.
• Supervision and reviewing new orders and MAR daily to ensure task is being completed timely and accurately.
• Evaluating, supervising, and reviewing the transcription process, medication and equipment ordering, and pharmacy cycle fill.
• Evaluating and supervising resident care, assessments (weight, skin, etc.), and medication passes as needed.
• Completing weekly skin checks and documentation.
• Completing your specific daily check list and all audits scheduled to be completed.
• Following Infection Control policies and procedures, as well as tracking all infections for residents and staff.
• Being available by phone after hours on an on-call basis, split with additional staff, to assist staff when there is a question or concern regarding a resident care or service.
• Participating in administrative meetings and other gatherings as deemed necessary.
• Reviewing ISPs and resident care notes in electronic medical record as well as those provided by the VP.
• Completing proper documentation in electronic medical record.
• Showing up for work on time and following the appropriate call-in procedure when necessary.
• Maintaining a high standard and professional dress attire.
• Attending all mandatory staff in-services.
• Performing other duties as assigned.
• Following rules and regulations outlined in DHS 83 and CSL Policies and Procedures that are not specifically outlined above.
Qualifications:
• Active Registered Nurse License in Wisconsin.
• Experience in CCU, ICU, Emergency Room preferred, but not required.
• Additional education in rehabilitative or geriatric nursing preferred, but not required.
• CBRF Registry, CPR/AED certified, and Alzheimer’s/Dementia care experience preferred, but not required.
