NOTICE OF VACANCY

POSITION: HEALTH SERVICES – REGISTERED NURSE (RN) 11-month – Part-Time

LOCATION: Infant & Toddler Program

POSITION SUMMARY:  The Infant and Toddler Program (ITP) nurse serves as a member of the individualized family service plan (IFSP) team. The ITP nurse utilizes the nursing process, which is the systematic application of knowledge and skills used in assessing situations, deciding the course of action, implementing the action, and evaluating the outcomes. The ITP nurse helps make decisions, under the direction of a physician, about how care is provided and who provides the care to the child. The ITP nurse manages the process of assessment, evaluation, maintenance and improvement of the health of children; strengthens and facilitates the educational process by improving and protecting the health status of children; participates in the identification of and assists in the removal or modification of health-related barriers to learning in individual children. The ITP nurse is responsible for case management of medically fragile infants and toddlers in the program. One of the primary roles of the ITP nurse is to support child development by serving as an advocate and liaison between the home, the school, and the medical community regarding concerns that may affect a child’s ability to learn. 

REPORTS TO: School health supervisor and Special Education director/supervisor

EDUCATION, CERTIFICATION AND EXPERIENCE:

At the time of application, the candidate must hold or be eligible for:

  1. Successful completion of an accredited nursing program for a registered nurse. Bachelor’s degree in nursing preferred
  2. Active license as a registered nurse in the state of Maryland
  3. A minimum of two years nursing experience
  4. Experience in pediatric nursing and public health nursing
  5. Current cardiopulmonary resuscitation certification for health care providers from the American Heart Association

KNOWLEDGE, ABILITIES, AND SKILLS:

  6. Demonstrated success in accomplishing tasks akin to those responsibilities listed below

ESSENTIAL JOB FUNCTIONS, DUTIES & RESPONSIBILITIES:

The ITP nurse’s role can be defined, but is not limited, to the following functions:

  1. Promotes and protects the optimal health status of children
  2. Provides case management, screening and evaluation services
  3. Conducts health assessments
  • Obtains a health and developmental history
  • Maintains contact with pediatrician, specialists and other medical providers to keep informed of child’s current health status
  • Observes the child for developmental patterns in making nursing assessment and nursing diagnosis
  • Identifies abnormal health findings
  1. Develops and implements a health and educational plan (IFSP)
  • Interprets the health status of children to parents and school personnel
  • Initiates referrals to parents, school personnel and community health resources for intervention, remediation, and follow-through
  • Provides ongoing health counseling with parents and school personnel
  • Recommends and helps to implement modification of educational programs to meet child’s health needs
  • Utilizes existing health resources to provide appropriate care to children
  1. Establishes a therapeutic relationship with child and families to promote positive outcomes for children
  2. Educates families about the Infant and Toddler Program and conducts home visits as appropriate, in accordance with state and local guidelines
  3. Develops and implements community outreach activities in order to increase public awareness of the Infant and Toddler Program in accordance with state and local guidelines
  4. Participates in the IFSP (individualized family service plan) process in compliance with state and federal guidelines including conducting assessments, setting child/family outcomes and providing early intervention services
  5. Maintains, evaluates, and interprets health data in order to meet individual needs of children
  6. Plans and facilitates the completion of school health management protocols for the child with special health needs
  7. Recommends provisions for natural environments and/or school environment conducive to learning
  8. Provides direct health education and health counseling to assist children and families in making health and lifestyle decisions including review of diagnosis, medical equipment, medical reports and medications.
  9. Serves as a resource person to Infant and Toddler Program staff concerning relevant health information
  10. Counsels families concerning problems, at-risk behaviors, mental health issues, and substance abuse in order to facilitate responsible decision-making practices
  11. Coordinates medical input from the child’s health care provider and involves the health care provider as appropriate
  12. Coordinates school and community health activities and serves as a liaison between the home, school, and community
  13. Serves as a link between IFSP teams, child’s health care providers, and school-based nurse to communicate relevant health information as children transition to school-based services
  14. Compiles and collects data from service providers for preparation of reports required by MSDE/MITP (Maryland Infant and Toddler Program), and enters data into data tracking system for support of the Infant and Toddler Program
  15. Collaborates with other ITP service providers to plan and implement the child’s program
  16. Attends regular Infant and Toddler Program staff meetings, and state meetings as needed
  17. Collaborates with summer program providers as health care needs warrant
  18. Assumes accountability for professional ethics
  19. Maintains competence in nursing and participates in continuing education programs (i.e., workshops, seminars, conferences, classes or courses, and certification or degree programs)
  20. Knows and understands the legal aspects of nursing practice and the impact as it applies to case management and coordination of care
  21. Performs nursing functions in an ethical and professional manner in accordance with the Maryland Nurse Practice Act
  22. Accepts responsibility and accountability for professional nursing practice in the Infant and Toddler Program
  23. Provides services with respect for human dignity
  24. Safeguards child’s and family’s right to privacy by maintaining confidentiality of records and communication
  25. Thinks, concentrates, and interacts positively with others
  26. Comes to work regularly and promptly
  27. Works under stress and meets all deadlines

OTHER DUTIES:

(Duties listed are not intended to be all inclusive nor limit duties that might reasonably be assigned.) 

Performs related work as required or assigned by the school health supervisor and Special Education director/supervisor or designee.

PHYSICAL DEMANDS:

Position requires significant periods of standing and walking and is performed in a typical school and home environment. Work requires light physical effort. Tasks may include assisting children and moving equipment. Position also requires periods of sedentary work.

UNUSUAL DEMANDS:

May be subject to flexible hours.

TERMS OF EMPLOYMENT:

Eleven (11) month position and salary commensurate with licensure applied to the salary schedule for support employees (Scale 028). Work during summer required. All employees are encouraged to have automatic payroll deposit.

Performance will be evaluated in accordance with the policy on evaluation as established by the Calvert County Public Schools.
