LPN Full Time in Personal Care

$5000 Sign on Bonus

$26-35 per hour based on experience

80 Hours biweekly 

11 pm to 7:30 am. includes every other weekend. 

Weekend and shift differentials offered. $1.75 per hour weeknights/ $2.75 weekend 

Job Summary:

This position is responsible for performing or overseeing the performance of duties related to the care and services to residents and assigning work for Resident Assistants who may be on duty to assure the residents have the best quality of life.

Essential Job Functions:

• Receive reports from previous shift.
• Responsible for the medication pass, charting medications, and proper accountability of medications on the unit.
• Take vital signs and weight as required and maintain related records.
• Record and maintain required documentation on shift activities and residents.
• Communicate with Doctors for orders and to give updates on medical condition.
• Assist with activities of daily living, showers, tub, whirlpool baths, and answer call bells as needed.
• Communicate with families regarding resident condition.
• Help residents to appointments or activities as needed which may include pushing wheelchairs.
• Participate in quarterly Support Plan Conference
• Complete assessments and Support Plans
• Provide wound care dressings and other treatments.
• Write up fax orders on the doctor’s order sheet.
• Assure residents receive mail.
• Plan and conduct activities before meals and remind and encourage residents to attend campus activities.
• Receive telephone calls and take messages.
• Order medications following department policies and procedures.
• Wash, dry, and store resident’s personal laundry and towels.
• Supervise Resident Assistant including assigning work.
• Restock supplies on unit.
• Find substitute schedule coverage for call-offs.
• Prepare paperwork for appointments, lab slips and review MAR’s monthly.
• Prepare lab slips.
• Maintain and clean medication cupboards, medication refrigerator.
• Respond to Alert One calls and other emergency house calls.
• Offer snacks and resupply snack items.

Requirements

Qualifications:

• High School Diploma
• Current LPN Licensure in PA
• Current CPR
• Prior experience with long term care preferred
