CalOptima Health is seeking a highly motivated an experienced Personal Care Coordinator, Sr. (Bright Steps) to join our team. The Personal Care Coordinator Sr (Bright Steps) will coordinate activities aligned with the Certified Perinatal Services Program (CPSP) and support CalOptima Health moms with trimester specific, postpartum assessments and infant assessments. The incumbent will ensure communication of the member's assessment and care plan with the member and the health care team. The incumbent will identify barriers to member's care and assist in improving these barriers for all levels of care. The incumbent will work closely with the health care team to ensure member access to timely services and coordination of care.

Position Information:

Department: Equity and Community Health

Salary Grade: 304 - $53,813 - $80,720 ($25.87 - $38.8077)

Work Arrangement: Partial Telework

**This position is eligible for telework in California.**

Duties & Responsibilities:

85% - Program Support

Participates in a mission-driven culture of high-quality performance, with a member focus on customer service, consistency, dignity and accountability.

Provides outreach to members to coordinate completion of trimester specific assessments, including postpartum following CPSP protocols.

Conducts telephonic assessments.

Collaborates with the health care team to ensure timely communication of member's clinical information.

Maintains documentation of member's case, care plan and communication with the health care team within CalOptima Health's medical management system and ensures appropriate documentation of all interventions.

Identifies the need for referrals and coordinates with community resources or behavioral health services as needed.

Facilitates warm transfers in accordance with the member's needs when appropriate and notifies member's care team of key event triggers.

Guides members in understanding and accessing the benefits they are entitled to under Medi-Cal.

Leads a team of Personal Care Coordinators (PCCs) which involves mentoring and training staff, providing management with PCC performance updates and facilitating team meetings.

Oversees the training and development of PCCs to enhance their skills with current practices and broaden their knowledge through education.

Fosters collaboration and communication among multidisciplinary teams and external partners.

Engages with community stakeholders to promote CalOptima Health's maternity services and care coordination.

Partners with the manager to ensure documentation and reporting is accurate with all PCC team members.

10% - Administrative Support

Assists the team in carrying out department responsibilities and collaborates with others to support short- and long-term goals/priorities for the department.

Maintains compliance with established departmental productivity guidelines.

Provides productivity metrics reports to supervisor as needed.

Works with CalOptima Health staff from other departments to expedite the resolution of member concerns.

5% - Other

Completes other projects and duties as assigned.

Minimum Qualifications:

High School diploma or equivalent required.

2 years of experience working with maternal and child health programs required.

An equivalent combination of education and experience sufficient to successfully perform the essential duties of the position such as those listed above may also be qualifying.

Bilingual in English and in one of CalOptima Health's defined threshold languages (Arabic, Farsi, Chinese, Korean, Spanish, Vietnamese) required.

Preferred Qualifications:

Certification from California Board of Medical Assistant or American Registry of Medical Assistants.

3 years of experience working with maternal and infant health programs.

Develop rapport and establish and maintain effective working relationships with CalOptima Health's leadership and staff and external contacts at all levels and with diverse backgrounds.

Work independently and exercise sound judgment.

Communicate clearly and concisely, both orally and in writing.

Organize, be analytical, problem-solve and possess project management skills.

Work in a fast-paced environment and in an efficient manner.

Manage multiple projects and identify opportunities for internal and external collaboration.

Motivate and lead multi-program teams and external committees/coalitions.

Utilize computer and appropriate software (e.g., Microsoft Office: Word, Outlook, Excel, PowerPoint) and job specific applications/systems to produce correspondence, charts, spreadsheets, and/or other information applicable to the position assignment.

Ability to speak (enunciate) clearly in conversation and general communication.

Hearing ability for verbal communication/conversation/responses via telephone, telephone systems, and face-to-face interactions.

Manual dexterity for typing, writing, standing and reaching, flexibility, body movement for bending, crouching, walking, kneeling and prolonged sitting.

Lifting and moving objects, patients and/or equipment 10 to 25 pounds

Work is typically indoors and sedentary and is subject to schedule changes and/or variable work hours, with travel as needed.

Employee will occasionally work outdoors in varied temperatures.
