Stanford Sierra Youth & Families is Hiring a Housing Navigator Coordinator, Wrap Near Sacramento, CA

*$1,000 Hiring Bonus, Additional $1,500 Spanish Speaking *
QUALIFICATIONS
Education & Experience
  • Experience as a consumer and/or parent/caregiver of a child having previously received services within the public systems, including: mental health, child welfare, or juvenile justice
  • Basic knowledge of Public Mental/Child Welfare/Juvenile Justice systems, and the ability to advocate on behalf of the clients in navigating and negotiating those systems
  • Knowledge of housing resources, subsidy programs, and the Continuum of Care
  • Experience working with individuals and families who have been at risk or experiencing homelessness is strongly preferred
  • Previous experience with advocacy, outreach and community resources strongly preferred
ESSENTIAL JOB FUNCTIONS
Position Specific
  • In consultation with the assigned case coordinator and the client served, determine housing barriers, preferences, needs and goals. May assist client in completing entitlement applications
  • Participate in CFT (Child and Family Team) meetings in family homes or in the community
  • Provide on-going feedback and training to staff on the importance of a family's voice and choice and cultural sensitivity in all aspects of their treatment planning
  • Participate in community trainings, workgroups, planning committees and task forces related to housing, mental health, advocacy and policy development
  • Completes and reviews VI-SPDAT scores and matches clients to housing type via Sacramento County system protocols
  • Provides linkage to housing subsidies and support services to prevent, divert from or resolve homelessness
  • Provide linkage and direct support with rent gaps, short term emergency hotel/motel needs, utility resources and any other fiscal housing support as defined by Sacramento County.
  • Maintains a relationship with exiting property managers/owners and establishes new relationships as needed
  • Establishes, updates, maintains, and communicates a list of available housing opportunities
  • Field inquiries from property owners/managers and listen to their needs to ensure submission of completed housing applications where appropriate
  • Assist clients in their search for housing, filling out rental applications, and understanding tenant rights and responsibilities
  • Maintain accurate documentation of service objectives and outcomes
  • Prepares related reports including, but not limited to: outcomes, successes, barriers, etc.
  • Shares related reports with leadership and contract monitors
  • Ensures effective service delivery by coordinating individualized housing plans with their case coordinator and treatment team

  • Performs all duties in a manner consistent with the principles and values of agency, while adhering to applicable professional codes of ethics, the agency's policies and procedures, contractor requirements, and regulatory requirements.
  • Model and communicate appropriate positive attitudes toward the agency's Mission, Vision, and Values
  • Work collaboratively with all agency programs and staff to provide support as needed
  • Utilize and maintain calendar with all work-related details in order to manage time effectively and share calendar information with coworkers
  • Participate in on-going training to expand and develop professional skills
  • Perform other duties as necessary for the agency, as assigned

Employment at the Agency is terminable at-will, which means that employment may be
terminated at any time, without cause or reason, by either the employee or the Agency. In
addition the Agency may also demote, layoff, transfer or reassign employees at any time at its
sole discretion without cause or reason.

Starting Salary Range: $25.00 - $30.00 DOE
