Caliber Management Group is Hiring a Remote Chronic Care Coordinator
Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse needed for Chronic Care Management company. Responsibilities included: Care Management, contacting patients, making referrals, ordering patient services, communicating with physicians and other health service providers. Experience using EHR required. Experience using eClinicalWorks EMR preferred. Experience in Neurology preferred. Must be comfortable making calls to patients. Ability to utilize technology required. This is a remote position.
Job Types: Full-time, Part-time
Pay: $17.00 - $19.00 per hour
Schedule:

- Monday to Friday
