Job Summary

The Licensed Practical Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Clinical Supervisor/Nursing
• Uses equipment and supplies effectively and
• Participates in personal and professional growth and
• Participates in the Performance Improvement activities as assigned by the Nurse Supervisor.

Qualifications and Skills

• Licensed Vocational Nurse with current licensure to practice in
• Has one year’s nursing experience and preferably in a Community/home health or medical/surgical
• Complies with accepted professional standards and
• Demonstrates good verbal and written communication, and organization
• Must be a licensed driver with proof of automobile insurance and is in good working
• Shows ability to organize and prioritize workload independently or with minor directions from
• Must possess basic knowledge of the use of mobile phones
• Must possess basic knowledge of computer

Prolonged or considerable walking or standing. Able to lift, positions, or transfer patients. Able to lift supplies and equipment. Considerable t aching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.

LVN to service these cities: -

Pomona; Diamond Bar; Walnut; Covina; West Covina; Eastvale; Corona; Rialto; Riverside. Also Redlands to Beaumont Cities. The assignment will be based on convenience to Nurses in Resident City.

Job Types: Full-time, Part-time

Pay: $30.00 - $38.00 per hour

Expected hours: 40 per week
