Shift:

General Description of the position:

The Medication Aide is responsible for medications, treatments and resident care duties as assigned by the Director of Resident Care Services or the Executive Director. This position administers medications according to acceptable standards. The Medication Aide engages in the practice of Nursing within the scope of the Nurse Practice Act under the guidance and delegation of a licensed Registered Nurse. If appropriate, answers questions of residents and their family members. Insures that the medication area is kept clean. The Medication Aide is responsible to ensure that the next shift has a responsible and qualified staff member on duty to administer resident medications.

Essential functions:

 Adheres to and conveys the community’s philosophy of maintaining elements of self-care and encouraging independence.

 Daily internal contacts with the Director of Resident Care Services.

 Read communication log daily.

 Report to changing shifts any changing resident needs or conditions, and any tasks needing to be completed or followed-up.

 Count narcotics with changing shift.

 Fills out Incident Report for resident and/or personnel when an incident occurs on shift.

 Takes and records vital signs as ordered and otherwise when necessary, i.e. monthly or daily as instructed by supervisor.

 Administers medication and treatments as assigned within the scope of practice.

 Maintains accurate, complete and confidential resident medication and care records, according to established procedure.

 Notifies the Director of Resident Care Services immediately when a resident becomes severely ill or injured.

 Keeps Med carts orderly, clean and appropriately stocked.

 Notifies the Director of Resident Care Services at once if noting any adverse reaction of medication on resident, or awareness of medication error.

 Observes and communicates to the Director of Resident Care Services any changes in resident physical or mental status.

 Communicates as needed with doctors, families, and other appropriate parties, concerning resident status, changes in status, need for appointment, etc. Documents conversations in communication logs.

 Confers with Physicians as to medication orders.

 Performs unlicensed treatments.

 Supervise assisted living aides to ensure safety and health of all residents.

 Chart appropriate information.

 Work under RN license to perform delegated tasks.

 Passes morning, afternoon, and evening nourishment/medications, staying with each resident until the nourishment/medication is consumed, then charts appropriately.

 As appropriate, provides leadership to other shift personnel

 Remains calm during emergencies.

 Assists in all care of residents, as needed.

 Demonstrates knowledge of infection control, i.e. adequate hand washing.

 Maintains upkeep and cleanliness of med carts and medication room.

 Responsible for recording and/or restocking of medications from pharmacy after receiving proper instructions. Responsible for locking and storing narcotics until turned over to pharmacy.

 Responsible for ordering cycle fill medications.

 Responsible for gathering and recording I & O’s.

 Assists in other nursing duties as appropriate and as assigned by supervisor.

 Responsible for ordering new and routine medications as ordered by physician.

 Answer telephones.

 Respond to emergencies.

 Follow HIPPA guidelines.

 Monitor emergency call lights.

 Monitor vital signs.

 Purge hard charts as directed.

 Filing.

 Mandatory abuse reporting to the Director of Resident Care Services.

 Understand the chain of command.

 Train new Medication Aides.

 Other duties as directed.

Routine Tasks:

 Finger sticks for diabetics.

 Peri-care on females (vaginal secretions).

 Handling soiled linen.

 Clean up of potentially contaminated fluids.

 Attending to an injury where blood is present.

I understand that because of the above-mentioned routine tasks, I could be at potential risk for contracting a bloodborne pathogen, including but not limited to non-A hepatitis, non-B hepatitis, hepatitis B (HBV), delta hepatitis, syphilis, malaria and/or human immunodeficiency virus (HIV).

Therefore, the company will provide me with the appropriate training in Universal Precautions and OSHA ‘s Exposure Control Standards in order to minimize any possible exposure. They will further provide me with an HBV vaccination if I so choose.

It will be my responsibility to adhere to all Exposure Control Policies as set forth by the company

I have read the above and have received training on Bloodborne Pathogens.

Working conditions:

 Sitting/Standing: up to 8 hours

 Lifting requirements: up to 200 lbs. with assistance.

 Phone time: up to 4 hours daily

 Computer time: up to 4 hours daily

Knowledge, skills and abilities required:

Must have excellent communication skills both written and verbal with Residents, their families, staff and Management Company and/or Owners.

Equipment used:

 Office equipment including but not limited to: telephones, computers, copiers and printers.

 Occasional tasks may require the use of (but not limited to) lifts and needles.

Qualifications:

 A genuine concern for the elderly.

 Must perform excellent customer service skills

 Ability to lead and motivate people.

 Capable of making mature judgments.

 Remain drug free.

 Well groomed.

 Telephone answering skills.

 Interpersonal relationship skills.

 Oral and written communication skills.

 Ability to manage and prioritize a large work load.

 Able to pass CPR.

 Must have a Multnomah County and/or Oregon food handler’s permit.

 Must have First Aid Training Certificate.

 Must have abdominal thrust training.

 Ability to maintain confidential information.

 Dependable.

Education:

High School diploma. At least six months on-the-job experience.
