Description
The Health Coach will provide individualized self-management training to individuals and groups according to the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
Health coach participates in facilitating individual and group education, reinforces self-management skills, supports behavior changes, provides psychosocial support, and ongoing self-management support to program participants.
Requirements
• Establishes a schedule for coaching contacts, which considers the complexity of health and functional status and includes the social support system.
• Collects assessment data in a collaborative and ongoing manner and document activities in electronic medical record system.
• Utilizes motivational interviewing techniques and collaboratively develops behavior goals with participant that are in alignment with the treatment plan; assesses progress toward goal achievement and modifies the education intervention accordingly or seeks assistance with barrier resolution/other identified issues from appropriate team member.
• Provides non-clinical educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
• Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
• Identifies when a program participant's needs are outside the scope of the paraprofessional’s scope of practice and refer to the appropriate professional experts.
• Contributes to the development of training materials.
• Contributes to, and participates in, a continuous quality improvement process to measure DSMES program and to identify and address opportunities for improvement.
• Consistently meets department productivity goals as defined by business needs and team metrics.
• Maintains 15 hours of continuing education annually specific to diabetes, diabetes related topics and behavior change and self-management education strategies.
KNOWLEDGE, SKILLS AND ABILITIES:
• Basic knowledge about diabetes and related chronic conditions.
• Able to lead and effectively manage groups.
• Ability to develop a collaborative, therapeutic alliance with individuals.
• Basic computer skills (use of Internet and e-mail).
• Bilingual in English and Chinese (Cantonese, Mandarin, or Fuzhou).
EXPERIENCE/EDUCATION:
• Bachelor’s degree in a health-related field preferred.
• Experience working with community-based groups and providing outreach to older adults and persons with disabilities preferred.
• Minimum of 6 months experience (preferably more) providing diabetes education and/or 15 contact hours within past year in diabetes.
• Health Coach Certification preferred.
• Medical Assistant Certification preferred.

Salary Range: $42,700-$60,000
