Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate. Provides assistance with personal hygiene and elimination needs according to the established plan of care. Assists patients with ambulation and transfer using proper body mechanics. Communicates any change in condition or lack of patient progress. Schedules visits with the patient or caregiver.
