Nurse Practitioner-Post Acute Care

Locations: Cape May, NJ

Shift: Full-Time, Monday- Friday

Salary: Competitive based on experience

Requirements:

• Possess a current license to practice as an Advanced Practice Nurse in the State of New Jersey.
• Two years of work experience as an Advanced Practice Nurse preferably with experience in post-acute care.
• Possess a valid driver’s license, valid auto insurance, and reliable transportation.
• Must maintain a professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisors, co-workers, and all others supporting the mission and vision of the organization.
• Must be able to demonstrate optimism, enthusiasm, and willingness to work constructively with other team members and reflect concern for the well-being of others.
• Possess satisfactory references and pass criminal background, national sex offender, drug and motor vehicle checks.

Responsibilities:

• Post-Acute Care Advanced Practice Nurse performs medical examinations and post-acute care based upon patient need.
• Assist in managing the patient's symptoms including prescribing medication or care by coordinating with referring providers.
• Document visits in the patient's clinical record.
• Provide medical care services and post-acute care intervention.
• Collaborate and communicate effectively with the Post-Acute Care Interdisciplinary team.
• Participate in the Quality Assessment/Performance Improvement Program of the agency as appropriate and needed.
• Exercise independent judgement, critically analyze situations, and make recommendations using appropriate problem-solving skills.
