Job Summary:
We are seeking a dedicated Wound Care Nurse to join our team at Bryan Manor. The ideal candidate will provide specialized care for patients with chronic or acute wounds, promoting healing and preventing complications.

Duties:
- Assess and monitor wounds to develop individualized care plans
- Clean and dress wounds according to best practices and physician orders
- Educate patients and caregivers on wound care techniques and prevention strategies
- Collaborate with physicians and healthcare team members to optimize patient outcomes
- Document all wound assessments, treatments, and progress notes accurately

Qualifications:
- Valid RN or LVN license (Wound Care Certification preferred)
- Previous experience in wound care management in settings such as nursing homes or acute care facilities
- Proficiency in wound assessment, debridement, and dressing techniques
- Knowledge of catheterization, dialysis procedures, venipuncture, and medication administration
- Familiarity with Dementia care, Durable Medical Equipment (DME), and Memory Care protocols
