Summary:
Coordinates the implementation and delivery of outreach initiatives and referral programs available to patients and families enrolled in the organization's program. Collaborates with cross-functional department teams to develop educational public awareness and community outreach programs to locate and connect with families who may be eligible to receive early intervention services. Develops relationships with community partners and key referral sources and ensures their knowledge of services and programs available.
