Our Dedicated MDS Coordinator of over 20 years is retiring! The BronxCare Special Care Center is seeking a skilled and experienced MDS Coordinator to join our team. As an MDS Coordinator, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for our residents/patients. This is a vital role in ensuring accurate and comprehensive patient care plans are developed and implemented.
Duties:
- Conduct comprehensive assessments of patients' medical, physical, and psychological conditions
- Collect and analyze patient data to determine appropriate care plans
- Collaborate with interdisciplinary team members to develop individualized care plans
- Ensure accurate and timely completion of MDS assessments in compliance with regulatory guidelines
- Monitor and update patient records to reflect changes in condition or treatment plans
- Provide education and training to staff on MDS documentation requirements
- Participate in quality improvement initiatives to enhance patient care outcomes
Qualifications:
- Valid Registered Nurse License
- Proven experience as an MDS Coordinator or similar role
- In-depth knowledge of MDS assessment process and regulations
- Strong understanding of infection control protocols
- Familiarity with Reliable EMR Software
- Excellent communication and interpersonal skills
- Ability to prioritize tasks and meet deadlines
- Basic life support certification
