5. Duties and Responsibilities
1. Provide post-release housing, healthcare and employment placement to inmates incarcerated at the Bristol County Sheriff’s Office as part of discharge planning.
2. Meet inmates and complete all necessary paperwork, such as intake assessment, post-release plan, contact notes, and update weekly/monthly database for the United Way, etc.
3. Aid in the assessment and post-release treatment planning of inmates including, but not limited to assisting inmates with accessing services during incarceration and post-release to the community by making referrals to various correctional programs and outside agencies that can provide assistance to the inmate upon release into the community with housing, healthcare, employment and other resources as needed.
4. Arrange for transportation for all inmates being released who are in need of transportation.
5. Attend and participate in community collaborative meetings outside of the Bristol County Sheriff’s Office that are relevant to the population.
6. Work in collaboration with community agencies assisting with prisoner re-entry.
7. Work in collaboration with community placements to ensure post-release plan is maintained and appropriate services are rendered.
8. Attend Bristol County Sheriff’s Office in-service training, as well as outside trainings relevant to the job of Post Release Worker.
9. Ensure adherence to Bristol County Sheriff’s Office policies and procedures, ACA and Department of Correction standards/regulations.
10. Perform other duties as designated by the Sheriff of his designee.

1. Good communications skills.
2. Good organizational skills
3. Ability to work in collaborative manner with corrections personnel, inmates, and community agencies.
4. Computer skills including Microsoft Excel, Microsoft Word and Internet Searches
5. Bachelor or Associate Degree Preferred
