Avenues by Brio Living Services
is a membership program that utilizes a holistic and innovative approach to aging in place. It is the only program in West Michigan that provides immediate support, guidance, and collaboration with a team of professionals for Seniors who wish to stay in their home. Future care is coordinated and funded through the membership plans.

Position Summary:

The Member Navigator role assists individuals (its members) in accessing and navigating healthcare and wellness in West Michigan area. The primary role is to development relationships with Avenues members and being their point of contact. The Member Navigator will provide advocacy, utilize technology to enhance support, coordinate medical and support services as members age in their home. The Member Navigator will engage with Avenues Members during times of wellness and respond to a health crisis with a network of support. This position becomes a trusted companion to our Avenues Members due to the comprehensive care we provide. This position report to the Executive Director of Avenues.

Essential Responsibilities:

Member Support

  • Provides one-on-one guidance and support to members. Assisting them in understanding and access to available services, benefits and resources in the healthcare and wellness.
  • Establishes rapport and a therapeutic relationship with the member, their families, and other healthcare team members.
  • Provide clear and concise information to members about their rights, options, and responsibilities regarding healthcare or wellness.
  • Increases member's and families' health and resource literacy (medication, resources, medical terminology, insurance coverage, and treatment options) to enhance their and empower them to make informed decisions.
  • Advocates on behalf of members to ensure they receive timely and quality care or services, addressing any barriers or challenges they may encounter.
  • Provides advocacy during times of medical crisis by communicating member desires and wishes with hospital staff, family members, and pertinent internal staff.

Assessment

  • Performs comprehensive assessments, identifies short and long term needs, and develops appropriate strategies and goals to address those needs.
  • Demonstrates an understanding of the many levels of care and actively provides support to the Avenues member.
  • Guides the transition through the health care continuum in provision with the membership agreement.
  • Utilizes available data to drive programs and services to optimize the emotional wellness of members

Follow-Up

  • Monitor and follow-up with members to track progress, address concerns, and make necessary adjustments.
  • Promotes smooth and timely continuity of care.
  • Coordinates caregiver and support staff in care delivery.
  • Participates in after-hour phone calls and/or after-hour home visits.
  • Monitor and engages with in home technology to supplement in-person care giving.
  • Fosters effective communication to ensure collaboration with all parties.

Documentation

  • Utilizes Electronic Medical Record keeping to effectively communicate changing member needs to other team members.
  • Maintains financial accountability for Avenues.

Community Engagement

  • Assists members with navigating community resources to promote successful aging in place.

Qualifications:

  • Bachelors degree preferred.
  • Experience in healthcare navigation, case management, or medical/health professional preferred.
  • Experience working with the senior population preferred.
  • Medical terminology proficiency required.
  • Ability to develop and implement a holistic, wellness based approach to health and wellness for Avenues Members.
  • Strong interpersonal and communication skills, including active listening and empathy.
  • Valid driver's license and access to transportation as well as auto insurance required.
