PURPOSE OF POSITION:

Plans, organizes, coordinates and delivers medically-related social services to attain or maintain the highest practical physical, mental, and psychosocial well-being of each resident. Enables and promotes staff to maintain or enhance each resident’s dignity in full recognition of each resident’s individuality. Coordinates discharge planning for residents who could benefit from a different level of care or who can return home.

PRIMARY DUTIES AND RESPONSIB ILITIES:

1. Assess and complete Social Work Assessments
2. Provide community resources
3. Thoroughly document records
4. Review MOLST with patients or and family
5. Confirm discharge planning
6. BIMS/PHQ9 completions
7. Individualized care plans
8. Complete MDS in a timely manner
9. Offer/Complete as needed Advanced Directive/POA paperwork
10. Psych referrals (recommend to SSD)

MINIMUM POSITION QUALIFICATIONS:

• EDUCATION: BA degree

 EXPERIENCE:

• At least one year of supervised social work experience in a health care setting working directly with individuals.

SPECIAL REQUIREMENTS:

• Ability to read and write English, perform arithmetic calculations, and communicate with patients, t6heir relatives, physician, and a variety of community personnel at a level normally achieved byobtaining a Bachelor’s Degree.

• Work involves limited physical activity as majority of work time is spent in an office environment.
