Position Summary:
The Targeted Case Manager will provide Targeted Case Management, assessment, and treatment planning services to patients with a substance use disorder. Targeted Case Management services mean those activities provided to assist and support individuals in gaining access to needed medical, social, educational, and other services essential to meeting basic human needs. Targeted Case Management services may include interactions with family members, other individuals or entities, and includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Targeted Case Management services at BrightView consist of: (1) assessment of the eligible patient to determine service needs; (2) development of an individualized integrated action plan; (3) referral and related activities to help the patient obtain needed services; (4) monitoring and follow-up; and (5) evaluation.
Job Responsibilities:
  • Connect patients and families with available resources and follow-up to document
  • use/success of referral.
  • Complete detailed reports on needs assessments and case management activities.
  • Input accurate and complete case management data into electronic patient database.
  • Assist patients in accessing the Ohio Benefits Bank and other appropriate medical,
  • social, educational, and employment resources.
  • Monitor and ensure patients are seeing health care professionals outside of Brightview
  • when required by the BrightView treatment team.
  • Provide crisis and de-escalation techniques.
  • Assure documentation is completed in compliance with contract regulatory
  • agency requirements and best practices.
  • Ensure patients have reliable, consistent transportation to BrightView and other care
  • providers.
  • Coordinate patient care with other BrightView team members.
  • Complete documentation within specified timeframe using Electronic Medical
  • Records.
  • Attend required meetings including individual and group supervision, treatment team
  • meeting, and quarterly in-service.
Job Qualifications:
  • Bachelor’s degree required "Behavioral science" means:(a) Psychology;(b) Sociology;(c) Social work;(d) Family studies;(e) Human services;(f) Counseling;(g) Nursing;(h) Behavioral analysis;(i) Public health;(j) Special education;(k) Gerontology;(l) Recreational therapy;(m) Education;(n) Occupational therapy;(o) Physical therapy;(p) Speech-language pathology;(q) Rehabilitation counseling; or(r) Faith-based education.
