BrightStar Care is currently seeking skilled and compassionate LPN's to provide high-quality one-to-one care in home care settings. Responsibilities:

● Provide skilled nursing care to patients in their homes
● Perform assessments, develop care plans, and implement appropriate interventions
● Administer medications, treatments, and therapies as prescribed
● Monitor and assess patient's condition, including trach and ventilator management
● Collaborate with the interdisciplinary team to coordinate patient care
● Educate patients and their families on disease management and self-care techniques
● Document patient care activities and maintain accurate medical records
● Adhere to all professional, ethical, and legal standards of nursing practice

Requirements:

● Valid LPN license in the state of New York
● Minimum of 3 years of clinical experience as an LPN in home care or critical care settings
● Experience and proficiency in trach and ventilator care
● Excellent clinical assessment and critical thinking skills
● Strong communication and interpersonal skills
● Ability to work independently and prioritize tasks effectively
● CPR certification
