Responsibilities:
Skilled Nursing Care: Deliver a range of skilled nursing services, including wound care, medication management, IV therapy, and other specialized treatments, in the homes of our patients.
Patient Assessment: Conduct comprehensive assessments to determine patients' needs and develop individualized care plans tailored to promote optimal health outcomes.
Collaboration: Work closely with a multidisciplinary team, including physicians, therapists, and other healthcare professionals, to ensure coordinated and comprehensive care.
