Schedule: Monday- Friday 8AM-5PM No on call required!
The RN Case Manager is a member of the IDG (Interdisciplinary Group) who coordinates hospice plan of care in coordination with the medical director and other members of IDG. Other duties include- Assessing and evaluating the patients' needs within their home or a facility setting, supervising Certified Nursing Assistants, physical assessment and symptom management, medication management, and ensuring all hospice-related patient needs are met.
Skills and Qualifications:
- License Registered Nurse in the state of GA or Multi State License
- CPR Certified
- Reliable Transportation
- Valid Drivers License and Auto Insurance
- Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention and family intervention
- Excellent verbal and written communication skills
- Excellent bed side manners
- Excellent time management skills
