The Trauma Outreach Manager (TOM) is a Registered Nurse with an expertise in the management of the trauma patient and trauma systems. The TOM reports directly to the Nurse Director, Trauma and Burn Program. This position serves as a central point of reference for information about all patients transferred into our program from the field as well as outside facilities. This individual will guide the development of administrative and clinical aspects of the program as it relates to our relationships with EMS agencies and outside facilities. S/he will be directing and/or collaborating with a diverse range of internal and external contacts, ranging from pre-hospital providers, patients/families, care improvement consultants, nurses, surgeons, and other care providers and multidisciplinary specialists, as well as medical directors. The scope of the position includes knowledge of: Commonwealth of MA EMS trauma entry and transport guidelines, complex trauma and burn patients, organization and integration of individual hospitals into the regional and state trauma system and quality and safety programs.

PRINCIPAL DUTIES AND RESPONSIBILITIES:Indicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation.

  • Provides an environment for optimal interfacility (contract and non-contract referral centers) clinical management and transfer of trauma patients.
  • Ensure an efficient system for the transfer of trauma patients from contract and non-contract hospitals to the BWH trauma center
  • Collaborate with identified clinical and administrative leadership at contract and non-contract referral centers to develop joint protocols for optimal care.
  • Develop, implement and evaluate approved policies, procedures and standards of care necessary for the delivery of excellent patient-focused care across the continuum of care from injury to definitive care.
  • In conjunction with the Trauma Medical Director and Nurse Director, responds to change in clinical practice by planning, designing, implementing and evaluating guidelines, protocols and standards in the management of trauma and burn patients.
  • Promote an environment that is sensitive to the culturally diverse needs of patients and their families.
  • Provide feedback to all EMS transporting agencies and referral centers on trauma and burn patients transferred to the BTS service at two points in time: initially within - 72 hours of transfer, and then upon discharge from the BWH.
  • Liaise with emergency medical services and pre-hospital providers to assess and provide critical clinical feedback on pre-hospital care to inform future care guidelines and pre-hospital care protocols and identify educational programming needs
  • Participate in a Multidisciplinary Peer Review and Trauma Executive Committees at the referral centers and at BWH .
  • Maintain knowledge of the external regulatory agency (American College of Surgeon and Department of Public Health Commonwealth of Massachusetts) requirements for verification and designation of trauma centers.
  • Develops, implements, and evaluates educational programs based upon assessed learning needs of patients, families, and staff.
  • Complete an initial educational gap analysis for MD’s, RN’s, Paramedics and EMT’s at or associated with contract referral centers regarding the clinical management of trauma patients.
  • Develop and deliver educational programming to address needs identified by the gap analysis for audiences including but not limited to physicians, nurses, and pre-hospital providers.
  • Conducts educational programs as directed by needs assessment and clinical and administrative recommendations from the outside facilities.
  • Coordinates internal and external Trauma Grand Rounds and other CME/CEU offerings
  • Supports the development of program-based injury prevention activities with analyses of commonly applied datasets, such as trauma, state or MA Department of Public Health, and nationally based (National Trauma Data Bank, as an example)
  • Assists Injury Prevention and Outreach Coordinator with program planning and presentation as needed
  • Leads and participates in the development and ongoing evolution of the trauma quality and safety program.
  • In conjunction with the Trauma Medical Director and Nurse Director, evaluate the outcomes of patient care including care delivered prior to arrival at BWH and provides constructive, respectful feedback to pre-hospital and transfer hospital providers.
  • Provide concurrent collection of patient care quality metrics for inclusion in the assessment of quality outcomes on all trauma patients. This includes following assigned cohort of patients through hospitalization, identifying hospital events as defined by the NTDB and internal quality filters, query providers regarding opportunities for improvement and assist in patient case write ups for faculty review at monthly interdisciplinary Peer Review conference
  • Under the direction of the Trauma Medical Director and Nurse Director establish corrective action plans for quality metrics that have been identified as failing to meet established standards of care.
  • Provide quarterly reports to Trauma Workgroup summarizing:

i. clinical activities specific to the contract referral centers

ii. quality metrics specific to the contract referral centers

  • Supports the activities directly related to the processes required to maintain American College of Surgeons Level I Trauma Verification and American Burn Association Burn Center Accreditation at BWH.
  • Demonstrates accountability for ongoing personal/professional development
  • Maintain and update clinical knowledge and skills based on current health care practices.
  • Identify own learning needs and goals, and design a plan to meet them.
  • Participate in activities on state, regional, and national levels that foster peer support and networking.
  • Fulfill professional role through involvement in professional organization activities, presentations, and publications.

Note: This role requires travel to referral facilities, as well as occasional early or late meetings and weekend meetings to accommodate clinician and community stakeholders’ schedules.

QUALIFICATIONS:

§ Bachelor’s Degree required and Masters degree in health-related field preferred (will consider candidates matriculated into Masters Program); current Massachusetts RN licensure required

§ 5 years of recent clinical experience in Emergency or ICU nursing required

§ Database management and analysis experience preferred

§ TNCC certification required within 6 months of hire; TNCC instructor required within one year of hire

§ ATLS coordinator certification within one year of hire required (training offered at BWH)

§ Experience in health care and patient safety quality measurement, reporting, and improvement preferred

§ Experience working in a large and complex health care organization preferred

§ Competence with MS Outlook, Word, Excel, are required for this position; candidate must be able to produce presentation materials including PPT slides and Excel spreadsheets.

SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:

§ Knowledge of current professional nursing theory, practice and skills with particular emphasis on the area of trauma and emergency surgery.

§ A demonstrated ability to fully participate in the planning, design, implementation and evaluation of guidelines, protocols and standards of care for trauma patients.

§ Provide education to clinicians at the referral facilities to include clinical applications, systems development and transfer priorities.

§ Highly motivated, independent and capable of performing in a complex environment; strong sense of initiative and perseverance to carry projects through to completion.

§ Excellent communication skills and the ability to work with all levels of clinicians and administrators.

§ Strong organizational skills

§ Is knowledgeable and compliant in all hospital, State and Federal regulatory requirements, including hospital policy and procedures (where applicable to the performance of the job), Joint Commission, and Health Insurance Portability and Accountability Act of 1996 (HIPAA).

§ The role will require occasional travel to referral facilities for onsite presentations, Grand Rounds, or consult visits
