Bridges of Hope Treatment Center is Hiring a Case Manager Near Anderson, IN

Summary:Case Managers at Bridges of Hope serve our clients in a variety of ways. Case Managers are generally responsible for coordinating the delivery of healthcare, including substance abuse treatment modalities.
Job Responsibilities (This list is non-exclusive):
  • Enhance the overall treatment process by assisting your clients and interdisciplinary team members to meet overall treatment needs of clients
  • Gather client personal information through self-report and collaboration with other providers
  • Evaluate client-specific needs and follow up by completing release of information forms, consents, disability paperwork, and FMLA documentation
  • Make and monitor referrals
  • Act as a liaison between a client and his/her friends, family, attorney, probation officer, employer, disability insurance, sponsor, provider, pharmacy, etc.
  • Collaborate with various agencies to set up individualized aftercare that keeps the client moving toward recovery
  • Document all interactions with, and on behalf of, clients in accordance with agency policy
  • Help to maintain clean and sanitary conditions throughout the facility
  • Participate in staff, treatment, and business development team meetings as appropriate
  • Keep electronic medical record up to date, including scanning documents to ensure complete client medical record
  • Engage in training sessions that offer learning opportunities that promote and enhance skill and professional development
  • Provide additional resources as needed (employment searching, applying for food stamps or health care, assist in getting he or she’s license back, applying for SS, etc.)
  • Maintain confidential information with tact and discretion beyond HIPAA
  • Other duties as assigned
Job Requirements:
  • High School Diploma or GED required
  • 2 years customer service experience, preferably in human services, possess demonstrated commitment to customer service
  • Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred
  • Must be over 21 years old
  • Excellent analytical and critical thinking skills with a high level of flexibility and diplomacy
  • Strong organizational and communication skills
  • Proficient in Microsoft office suite
  • CPR/ BLS certification within (60 days of hire)
  • Valid driver’s license required
  • Proof of insurance
Physical Requirements:
  • Mobility - Twist, Bend, Stoop/squat, Move quickly (respond to emergency), Climb stairs, Walk, Stand
  • Hearing - Hear normal speaking level sounds (person-to-person), Hear alarms (monitors, fire alarms, code alarms)
  • Visual - See information up to 24 inches away (monitors, computer screens, skin conditions), See objects up to 20 feet away (patient in room), Use depth perception, Use of peripheral vision.
  • Strength - Squeeze with hands (operate fire extinguisher)
  • Other - Reach above shoulders/overhead, Reach below the waist (electrical plugs, etc.), Reach in front (assist patient with arms out), Pick up objects with hands, Write with pen, Use computer (type), Pinch/pick or otherwise work with fingers (use keyboard, etc.), Twist at waist, Twist wrists/hands (turn objects/knobs using hands).
