SUMMARY:
The Licensed Practical/VocationalNurse is responsible for providing direct patient care under the supervision ofa registered nurse. Responsibilitiesinclude following the plan of care, providing treatments, and workingcollaboratively with the members of the team to help meet positive patient careoutcomes

Responsibilities of the Home HealthLPN/Vocational Nurse include, but are not limited to, the following:
- Providesdirect patient care as defined in the State of Wisconsin Nurse Practice Act.
- Implementsplan of care initiated by the registered nurse.
- Providesaccurate and timely documentation consistent with the plan of care.
- Assesses andprovides patient and family/caregiver education and information pertinent todiagnosis and plan of care.
- Participatesin coordination of home health services, appropriately reporting the identifiedneeds for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to theregistered nurse and/or Clinical Supervisor.
- Usesequipment and supplies effectively and efficiently.
- Participatesin personal and professional growth and development.
- Performsother duties as assigned by the registered nurse.

QUALIFICATIONS
To perform the job successfully,an individual should demonstrate the following competencies to perform theessential functions of this position:
- Graduate of an accredited practical nurse or vocational nursing program.
- Has three years nursing experience. Community/home health or medical/surgical experience is preferred.
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Illinois.
- Complieswith accepted professional standards and practice.
- Demonstratesgood verbal and written communication, and organization skills.
- Mustbe a licensed driver with an automobile that is insured in accordance withstate and organization requirements and is in goodworking order.
- Possessesand maintains current CPR certification.
