Intellectual and Developmentally Disabled (IDD) Program

GENERAL NATURE OF JOB DUTIES

Bridge Health is seeking an individual to join our Intellectual and Developmental Disabilities Team in the role of Case Manager. The duties of this role will include completing annual Individual Support Plans (ISPs), completing required documentation as needed to remain in compliance with DBHDD and other outside entities, completing face to face residential visits, checklists, and scanning and uploading documents as needed/required into the electronic health record (ECR). Works in coordination with Host Home Providers, Back Up Staff, and Support Coordinators to ensure that person centered approach to care is maintained. Primary work schedule is Monday through Friday; however, flexibility of schedule is a must.

MINIMUM QUALIFICATIONS

At Least 21 years old, three years of good driving history, as well as, good computer skills, ability to work independently and be highly organized.
