Primary Responsibilities

• Contacts various Physician offices and schedules follow up appointments for Residents as needed.

• Communicates with Resident and/or family member of scheduled appointment times.

• Safely transports residents to and from follow up appointments in community.

• Maintains cleanliness of facility van.

• Performs any miscellaneous work assignments as may be required.

Maintains a dependable attendance record and adheres to standards of cleanliness, grooming, hygiene, and dress code. Must have valid Michigan drivers license in good standings.

Education

• High School Diploma, preferred.

Experience

• Not required. CENA certification preferred

Physical Demands

• Primarily sedentary in nature with lifting less than ten pounds; reaching, handling, fingering, near-visual acuity, talking and hearing. Must be able to assist with transporting Residents in wheelchairs in and out of Facility van as well as in and out of Physician Offices/Clinics.

Working Conditions

• Inside in well-lighted, well-ventilated areas. Outside in all seasonal weather conditions.
