Briarwood Manor is currently looking for an RN (UAS-NY trained a plus) who would coordinates the overall clinical program of the LHCSA / ALP in conjunction with the Administrator, Residents’ Physicians and other service agencies.

• Completes nursing assessments of residents referred for services, identify residents’ needs, consult with residents’ primary physicians and initiates care plans.
• Facilitates on-going re-assessments of residents’ needs as determined by changes in resident’s status or as required by New York State Health Department regulations and agency policy.
• Supervises home care staff with the annual in-service training calendar
• Completes competency evaluations and annual performance evaluations for all home care staff.
• Counsels and disciplines all home care staff as required.
• Acts as the on-call nurse, as required, to respond to clinical problems.
• Responsible for assuring that resident care practices / policies are followed.
• Provides sufficient staff to assure uninterrupted service to residents at all times.
• Implements and keeps current a system that identifies due dates for resident’s medical evaluations & UAS updates and Care Plans.
• Completes and maintains thorough supervision documentation for each HHA and PCA..
• Directing / Monitor, the work of all ALP staff.
• Ascertain that each resident has a care plan and that the ALP staff is aware of and following it.
• Insure that Staff are providing medications, diets, activities of daily living and safety measures for ALP Residents.
• Completes and maintains thorough documentation for each ALP Resident on all aspects of care including the care that is completed by outside providers.
• Respond to pharmacy and medical providers’ with regards to recommendations and any other resident care issues.
• Valid and Current New York State Registered Nurse License and Registration.
