Briar Place Nursing and Rehabilitation is Hiring a Psycho-Social Assistant Near Indian Head, IL

JOB SUMMARY: The purpose of this position is to conduct and document group and one-to-one programs, meeting the psycho-social needs of the residents and demonstrating progress toward their goals.

QUALIFICATIONS:

  • Must have a high school diploma.
  • Demonstrate adequate English oral and writing skills.
  • Must have a high level of organizational skills, initiative, creativity, and communicate well with others.

JOB REQUIREMENTS:

  • Must be physically and mentally capable of performing routine, repetitive job duties.
  • Uses skill and understanding in handling residents.
  • Be considerate, pleasant, and professional in the performance of his/her duties.
  • Must have compassion, tolerance, and understanding for the elderly.
  • Knowledge of JCAHO, OBRA, IDPH, and HFS regulations.

MAIN DUTIES:

  • Support the facility's philosophy of care and strive to achieve its goals and objectives.
  • Meet with groups three (3) times a week according to schedule.
  • Carry out one-on-one intervention three (3) times a week according to schedule.
  • Follow schedule for group and one-to-one programs.
  • Write monthly progress notes in a timely fashion.
  • Chart daily attendance in group activities.
  • Prepare and complete monthly lesson plans.
  • Follow through with approach and goals developed.
  • Identify and communicate with Psycho-Social Director regarding any needed change in goals, programs, schedule, etc.
  • Complete care plan as needed.
  • Organize paperwork including:
  • a) schedule of program meeting times and places
  • b) list of one-on-ones and group members
  • c) group protocols
  • d) attendance
  • e) current goals or copies of evaluations and reassessments as needed
  • f) lesson plan
  • Assure that work schedules are followed.
  • Maintain the confidentiality of resident information and honor his/her personal and property rights.
  • Attend Psycho-Social department meetings and others as directed.
  • Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed.
  • Turn in Day Off Request forms, supply sheets, and annual medical examinations in a timely fashion.
  • Follow established fire, disaster, safety, infection control, and evacuation policies and procedures.
  • Perform other duties as assigned by the Director of Psycho-Social Services and Administrator.
