In this role, you will provide direct patient care, develop individualized care plans, assess patients, and documents changes in patient’s physical and mental status. Responsibilities:

• Coordinate the delivery of care and consistently demonstrate competency with technical nursing skills.
• Collect patient history and develop appropriate care plans.
• Document treatments and progress toward desired outcomes.
• Administer medications to patients per MD’s orders.
• Educate patients, caregivers, and families on how to manage care appropriately.

Qualifications:

• Active PN license.
• 1 years of experience.
• Valid driver’s license.
