The Admissions Coordinator supports our mission and values by working with residents, families, staff, and referral sources from the community on the preadmission and admission process to ensure a positive/smooth flowing admission process is achieved.

The Admissions Coordinator's primary responsibilities will include:

  • Protect and maintain confidentiality of resident's information and resident's rights.
  • Maintain the standards of nursing care and implements the policies and procedures of the nursing department.
  • Maintains current knowledge and skills with respect to admissions, insurance carriers, nursing procedures and technology.
  • Conducts pre-admission screening on all potential residents.
  • Responsible for census development at the Health Care Center. This includes initiating regular calls to potential referral sources during times of low census.
  • Supports ambitious census goals and guidelines regarding admissions and ensures that they are met.
  • Acts as a liaison between the facility and family members during the pre-admission and admission process. Upon admissions, refers family members to appropriate Department Head depending on specific need.
  • Handles all tours, admission screenings and initial inquiries.
  • Works with the Director of Nursing to assure that all admissions are screened thoroughly and adequately prior to admission.
  • Completes admission paperwork with the responsible party, family member, power of attorney or resident upon admission.
  • Ensures that the admission packet is completed in a timely manner and that all forms are presented and delivered in a neat, organized and timely manner.
  • Participates in Utilization Review daily.
  • Cultivates external relationships and networking contacts which foster community support, participation in activities/health care events, and utilization of healthcare programs.
  • Actively works to improve working relationships with referral sources to increase the quality and number of leads.
  • Measures the effectiveness of the inquiry/admission processes, and designs and implements improvements if necessary.
  • Ensures compliance with local, state, and federal rules and regulations.
  • Keeps staff well informed of pending admissions.
  • Has a strong working relationship with the therapy department to ensure that managed care updates are completed in the allotted timeframe.
  • Makes visits/contacts with the hospitals to keep open communication about room availability.
  • Makes initial contact at the hospital with prospective admissions.
  • Must be able to be flexible with schedule which includes working extended hours.
  • Participates in weekly Health Care Center leadership meetings and monthly QAPI meetings
  • Ensures proper case management and updates are submitted.
  • Show kindness and respect to all residents
  • Other duties as assigned.

What is required in this position?

  • Current LPN or RN license in the State of Ohio required.
  • Proficiency with Microsoft Office Suite required.
