Brentwood Hospital is Hiring an Adolescent Social Worker /Counselor (PLPC, LPC, LMSW, LCSW) Near Shreveport, LA

The Counselor/Social Worker will develop and coordinate individualized discharge plans for patients and to provide quality counseling care directly and indirectly to patients. Full time and PRN needed, premium weekend rate for Saturday and Sunday.

Responsibilities include:

  • Develops and coordinates an individualized discharge plan for the patient by utilizing treatment team and chart information to determine the patient's aftercare needs.
  • Identifies and assesses family or community resources such as, group homes, or mental health practitioners to meet patients' aftercare needs.
  • Communicates with referral sources and other appropriate parties regarding patients' progress.
  • Develops and implements a plan of treatment, in conjunction with members of multi-disciplinary team.
  • Conducts didactic and other appropriate patient groups.
  • Coordinates and implements patients' discharge.
  • Attends and participates in department and other program or clinical and administrative meetings.

Job Requirements:

This opportunity is available to individuals who meet the following requirements:

  • Master’s Degree in Counseling or Social Work REQUIRED
  • PLPC, LPC, LMSW, or LCSW licensure REQUIRED
  • One year experience working in a Mental Health Setting REQUIRED
  • Effective oral and written communication skills and knowledge of telephone etiquette is required
  • Ability to interact in a positive manner and to communicate well with co-workers, supervisor, and other hospital employees
  • Must be able to work in a fast-paced environment with frequent interruptions
  • Ability to maintain information as highly confidential
  • Able to exert up to 10 pounds of force occasionally
  • Ability to stoop, kneel, crouch, crawl, reach, stand, walk, push, pull, lift & grasp
  • Successful completion of Crisis Prevention Intervention (CPI) within 90 days of employment
