Breaking Ground is Hiring a Case Manager, Aging Services Near Bronx, NY

Reporting to the Clinical Coordinator (or Director), Aging Services, the Case Manager, Aging Services Team provides works to improve the quality of life and health for Breaking Ground’s older adults, including those who have previously experienced homelessness and may have mental illness, substance use disorder, and/or other chronic conditions by coordinating with primary social service providers and providing effective programs and services that enhance their ability to age in their home and community. The Case Manager provides auxiliary, short- to medium-term, solutions-focused services that support older adults in aging safely in the community by providing information and referral services, entitlements, benefits application and navigation assistance, housing retention services, resource development support, and community integration programming.This position has a 35-hour work week. ESSENTIAL DUTIES AND RESPONSIBILITIES:Carry a caseload of 25-30 participants for auxiliary, short- to medium-term, solutions-focused services Engage participants on caseload on a regular schedule based on level of need, with at least one contact per participant every 2 weeksConduct structured assessments to evaluate service needs and develop appropriate person-centered service plan based on assessmentProvide accurate and appropriate information on benefits, entitlements, programs, and case assistance and assist in accessing services, including referral, counseling participants and advocating and advocating on behalf of participants.Arrange for provision of services needed by participantsMaintain complete and accurate case records by documenting engagements, attempted engagements, referrals, and collateral contacts timely in electronic case management record or through other means Prepare reports and other documentation related to service provision and disseminate information when necessaryEscort participants to appointments as needed or appropriate*Collaborate with other Aging Services Team members to implement group programming, including potential co-facilitation of socialization or similar groupsConduct home visits as necessary to conduct assessments and/or engagementsCollaborate with external partners and site-based social services teams, including attendance at case review and case conferences, to ensure coordinated care for participants.Provide supportive counselingPerform other related duties as assignedMINIMUM QUALIFICATIONS:Bachelor’s degree or equivalent experience1-2 years of experience working with older adults and/or populations who have experienced homelessnessHousing-based case management experience strongly preferredExperience using electronic case management or health records preferredProficiency in solutions-focused interventions and motivational interviewing techniques preferred Superb collaborative skillsCreativity, strong problem-solving skillsDemonstrated success in working in a fast-paced environmentAbility to communicate (verbally and written) with diverse populations and stakeholdersAbility to travel throughout the 5 boroughs via public transportation or vehicleValid NY or NJ driver’s license with a good driving record, as defined by Breaking Ground is preferredBi-lingual Chinese, Korean, Russian or Spanish preferred Compensation: $24.73 - $24.73 / Per HourCompensation: $45,008.60 - $45,008.60 / Per Annum As part of our continued efforts to maintain a safe workplace for employees and clients, and in compliance with the City of New York COVID-19 Safety Requirement Mandate, Breaking Ground strongly recommends all employees, interns, volunteers, and subcontractors to receive an FDA authorized and/or approved COVID-19 vaccination and provide a one-time verification of immunization as a condition of employment.
